There is an obligation of physicians and staff to protect the confidentiality and security of patient health information;
We have policies and procedures to obtain signed confidentiality agreements from individuals who have access to patient health information, whether staff or third parties;
We restrict access to Personal Health Information unless access is required for a purpose authorized by The Health Information Protection Act (HIPA);
We have clear policies and procedures for patients to access and obtain copies of their records or change errors/omissions in records if applicable;
We have policies and procedures for the collection, use, disclosure & modification of Personal Health Information;
Our policies and procedures are put in place to protect the integrity, accuracy, and confidentiality of patient health information and to protect the security, integrity or loss.
